1. Fill out and mail the return merchandise authorization form to SupraSport.com.

Or email this form to Jessica@SupraSport.com

2. A SupraSport.com representative will reply with an approval or denial and an RMA number

3. When approved, return merchandise to SupraSport.com along with the completed RMA form that was returned to you from step 2
